Qualiopi certification is valid for three years, but it is no quiet entitlement: a surveillance audit must take place between the 14th and 22nd month after issuance. What is the surveillance audit for?
Its purpose is simple: verifying that your quality system has actually worked since certification. Where the initial audit accepted, for a new entrant, "ready-to-use" mechanisms (prepared questionnaires, a structured but blank improvement plan), surveillance demands application evidence: questionnaires genuinely collected and compiled, watch logs fed over the period, an improvement plan with closed loops, a living complaints register.\n\nIt is the appointment where the "new entrant" accommodation is settled: everything the initial auditor validated "on design" will now be checked "on execution".
What is checked first
- The handling of your initial non-conformities. If the initial audit raised minor non-conformities, their corrective actions are verified first. An unimplemented correction turns the minor into a serious problem.
- Criterion 7's "operating" indicators: feedback collection (indicator 30), complaints handling (31) and continuous improvement (32) are almost systematically examined — that is where a living system shows.
- A sample of indicators from the other criteria, at the auditor's choice, with particular interest in the watches (23-25), disability (26) and recent session files.
- Your organisation's changes: new services, new action categories, new contributors or subcontractors — every scope change should have been notified to your certification body as it happened.
On site or remote?
The surveillance audit very often happens remotely, over a shorter duration than the initial one (half a day to a day for a small structure). Conditions depend on the certifier and your profile. Prepare the logistics like a full audit: smooth screen sharing, a tidy digital evidence binder, full availability on the slot.
The classic 18-month traps
- The Potemkin system: everything was impeccable at the initial audit, nothing has run since. It is the first cause of surveillance non-conformities — and it shows within ten minutes (document dates, empty logs, compilations stopping at certification month).
- Questionnaires collected but never used: answers sleeping in a survey tool with no compilation or action. Indicator 30 passes, 32 falls.
- The watch reconstructed the week before: thirty entries dated the same day fool no one.
- Stale published rates: your site still shows "98% satisfaction (2024)" while your 2025-2026 compilations exist. Update before the audit.
- Undeclared changes: new address, new activity, strong growth — your certifier must have been informed.
